Clinical and Molecular Epidemiologic features of first imported ZIKV cases with familial aggregation in Guangdong, China

Objective To investigate the clinical and epidemiologic features of the imported Zika virus infection cases within one Chinese family flying from Venezuela to Guangdong.
Method Four members (two parents and two children) of a Chinese family from Venezuela were positive for ZIKV infection when the youngest son was first found fever at airport immigration inspection. They were isolated temporally in a local hospital in Enping city, Guangdong province, and clinical data were recorded and urine and saliva were collected to isolate ZIKV and obtain viral sequences. Envelop gene sequences of four individuals in our study were used to align with other reference sequences using Clustal 2.1 software. Reference sequences were selected according to the highly similar sequences BLAST (Megablast) scores. Phylogenetic trees were drawn using the Maximum likelihood method by Tamura-Nei model with gamma-distributed evolutionary rates in MEGA 7.0.
Results All of them except the mother presented the mild syndromes of rash and fever. Sequence alignment showed that the envelop gene sequences from the father, the daughter and the son were completely identical. Phylogenetic analysis demonstrated that this strain is similar to the several imported strains reported in recent months, which are all clustered into groups isolated from 2015 ZIKA outbreaks in Brazil.
Conclusion Our subjects are imported cases from Venezuela, in which they were probably transmitted by one single mosquito, but the possibility of direct contact infection cannot be ruled out. Epidemiologists and public health officials should be aware of the risk of further expansion of ZIKV transmission by local competent vectors.
