OBJECTIVE: Arteriosclerosis and atherosclerosis were two critical subclinical target organ damages, bearing significant prognostic value. Recent investigations indicated that, in the elderly, Pulse wave velocity (PWV) and carotid intima-medium thickness (CIMT) acted as an important biomarkers and provided predictive value of cardiovascular events and mortality, and were also tightly associated with other target organ damages (TODs). As arteriosclerosis and atherosclerosis were associated with PWV and CIMT respectively, the present study aimed to investigate the relationship between arteriosclerosis/atherosclerosis and TODs in the elderly.
METHODS: From June 2014 to August 2015, a total of 1599 community-based elderly subjects (age >65 years old) located in the northern Shanghai were recruited. The carotid-femoral pulse wave velocity (cfPWV), left ankle brachial index and other TODs indicators including the ratio of the early ventricular filling velocity (E) to the peak velocity of tissue doppler velocity of septal mitral annulus (E/Ea), carotid intima-medium thickness (CIMT), plaque in right carotid artery, left ventricular mass index (LVMI), modification of diet in renal disease (MDRD), urinary albumine-creatinine rate (UACR) were assessed on each participant.
RESULTS: Arteriosclerosis and atherosclerosis were significantly and independently associated with age, BMI, smoke, systolic BP and blood glucose (p<0.01). In the variance analysis, Arteriosclerosis and atherosclerosis were significantly associated with E/Ea (P=0.043). Logistic regression analysis showed that the left ventricular diastolic dysfunction (LVDD) was significantly associated with atherosclerosis (odds ratio [OR], 1.55 [95% Confidential interval [CI], 1.11-2.17], P<0.05) while microalbuminuria (MAN) was tightly associated with arteriosclerosis (OR, 1.84 [95%CI, 1.04-3.24], P<0.05).
CONCLUSIONS: In the community-based elderly, LVDD was strongly associated with atherosclerosis while MAN was strongly associated with arteriosclerosis.
