Predominant Characteristics of ESBL-producing Escherichia coli Isolates from Patients with Pneumonia in Guizhou Province in China

Objective: The aim of the investigation was to target analysis the predominant characteristics of ESBL-producing Escherichia coli (E. coli) strains from pneumonia patients in Guizhou province.
Methods: From January 2016 to February 2017, 69 of 237 E. coli strains producing ESBL from pneumonia patients were collected from four tertiary hospitals from Guizhou province. Phenotypic methods and antibiotic susceptibility tests were employed to characterize 69 ESBL-producers. The genotype of ESBL were analysed by amplification of PCR, direct sequencing with various set of PCR primers and the basic local alignment search tool online. Sample cluster analysis of the detection results was also performed.
Results: Among the 69 analyzed isolates, 69 (100%) harbored blaCTX-M, with in which the most predominant ESBL gene was CTX-M-14 (49/69, 71%), followed by CTX-M-15(17/69, 25%) and CTX-M-27 (15/27, 22%). Also, 61 (88%) harbored blaSHV and only 1 isolates carried blaTEM-1. More than one ESBL combination was detected in 43 isolates (62%). Forty-nine (71%) isolates demonstrated the multidrug resistant (MDR) phenotype. The sample cluster analysis showed that the multiple clone spread existed in this group of E. coli.
Conclusion: blaCTX-M is the most prevalent ESBL gene in ESBL-producing E. coli clinical isolates in Guizhou province and the dominant genotypes is blaCTX-M-14. The increasing prevalence of E. coli isolates with MDR phenotype contributes to clinical challenges and the consequent surveillance of ESBL-producing organisms is of paramount importance.
