Potential Circulating Biomarkers for Detection Cognitive Impairment in Dialysis Patients

Aim Present study is to examine the relationship between circulating miRNAs, uremic toxin levels, nerve injury biomarkers and cognitive impairment (CI) in dialysis patients.
Methods CI dialysis patients (peritoneal dialysis 5, hemodialysis 6) confirmed by mental status examination were enrolled for study. Another 8 hemodialysis patients with normal kidney function were recruited for control. Blood examinations included circulating miRNAs (next-generation sequencing, quantitative reverse transcriptase polymerase chain reaction), small water-soluble solute; asymmetric dimethylarginine (ADMA); protein-bound solutes; p-cresyl sulfate (PCS) and indoxyl sulfate (IS); middle molecules; interleukin 1-β (IL-1β), interleukin 6 (IL-6), tumor necrosis factor-α (TNF-α) and nerve injury proteins, neuron-specific enolase (NSE), heat shock protein 70 (HSP 70), S100B.
Results Eleven dialysis patients with CI, mean age 69 years, male 4, female 7 and eight controls, mean age 61 years, male 4, female 4 were enrolled for analysis. The levels of IL-1β, IL-6 demonstrated significant higher in CI patients than those in controls (35.12 vs. 13.88 pg/mL, p=0.013. 64.78 vs. 29.48 pg/mL, p= 0.015, respectively). However, nerve injury proteins did not show significant differences between CI patients and controls. Four miRNA sets demonstrated more than 1.5-fold-change in expression level in the blood of CI patients compared to those in controls, has-miR-182-5p, has-miR-144-3p, has-miR-451a, has-miR-486-5p.
Conclusions Present study showed CI dialysis patients demonstrated higher blood levels of middle molecules. Four miRNA sets, has-miR-182-5p, has-miR-144-3p, has-miR-451a, has-miR-486-5p, were the potential biomarkers to detect CI in dialysis patients.
