Influence of bicarbonate/lactate and lactate peritoneal dialysis fluid on peritoneal mesothelial cells

Purpose Several components of peritoneal dialysis fluid (PDF) are known to induce peritoneal injury. Lactate which is one of these bioincompatible factors have reported to suppress the phagocyte function in peritoneal leukocytes and macrophages, however its influence to peritoneal mesothelial cells is not well investigated. Bicarbonate/lactate PDF has been on the market in Japan recently, and we aimed the difference of influence of lactate(L) and bicarbonate(B) on peritoneal mesothelial cells.
Methods We used cultured peritoneal mesothelial cells from the omentum of Sprague- Dawley rats.
We made 5 groups and cells were stimulated each fluid as follows,
Control group; DMEM /F12 with 10%FBS
B/L-PDF group; B/L-PDF (1.36% glucose Reguneal®; Lactate 15mEq/L + Bicarbonate 25mEq/L ) with 200mM glutamate +10%FBS
L-PDF group; L-PDF (1.36% glucose Dianeal-N®; Lactate 40mEq/L) with 200mM glutamate +10%FBS
B/L group; Lactate 15mEq/L + Bicarbonate 25mEq/L + DMEM/F12 with 10%FBS
L group; Lactate 40mEq/L + DMEM /F12 with 10%FBS
(PH of all groups were 7.2)
After 72 hours stimulation mRNA of Cancer antigen(CA)125, Interleukin (IL)6, α-smooth muscle actin(SMA), E-cadherin, collagen type 1 and 3 were evaluated using real time polymerase chain reaction (each mRNA level was normalized to GAPDH mRNA level).
Results CA125 mRNA levels of all groups were same. IL6 mRNA were 1.55, 1.21, 1.42, 0.84, 1.54, αSMA mRNA level were 0.7, 0.97, 0.75, 0.83, 0.7, E-cadherin mRNA level were 1.73, 1.55, 1.72, 1.57, 1.8, collgen type 1 mRNA level were 0.85, 0.9, 0.73, 0.8, 0.76, collagen type III mRNA level were 0.65, 0.9, 0.68, 0.86, 0.7 in control, B/L PDF, L PDF, B/L and L group respectively.
Conclusion From these results, lactate may inhibit cell viability and induce epithelial mesenchymal transition and fibrotic change in peritoneal mesothelial cells.
