Risk Factors for Retroperitoneal Leakage in Peritoneal Dialysis Patients

Background Acute ultrafiltration failure (AUFF), characterized by a sudden reduction of ultrafiltration, is a serious clinical issue among long-term peritoneal dialysis (PD) patients and can lead to fluid overload and uremic symptoms. Retroperitoneal leakage (RPL) is a common cause for AUFF. In this paper, we analyze the risk factors for RPL in peritoneal dialysis patients in our center and obverse the outcome of conservative treatment following RPL.
Methods RPL was confirmed by magnetic resonance peritoneography (MRP) in patients diagnosed of AUFF after excluding mechanical problems concerning peritoneal catheter. PD starting time-matched patients were chosen as controls in the ratio of 1:8. Demographic and PD related characteristics were analyzed between these two groups and treatment outcome was observed in RPL patients.
Results During a 16-year period, 57 PD patients developed AUFF, and 32 (56.1%) of them were diagnosed with RPL confirmed by MRP. 17 (43%) RPL occurred in the first 3 months of peritoneal dialysis, while 8 (25%) occurred after 2 years. None of the RPL patients was complicated with hernia, pleural fistula or peritoneal dialysis tube exit fistula. The percentage of male patients was significantly higher in PD patients with RPL than in PD patients without RPL (24/32 vs. 133/256, P=0.016), and nulliparity was identified as a risk factor for RPL in female PD patients (4/9 vs. 6/116, P<0.001). After 2 months for intermittent peritoneal dialysis (IPD), all RPL patients improved remarkably confirmed by MRP except for 1 patient complicated with scrotal hydrocele and turned to hemodialysis.
Conclusion RPL is a common cause for AUFF. Masculinity and nulliparity are risk factors for RPL in PD patients. Temporary IPD is an efficient treatment for RPL.
