Emerging concept for coronary atherosclerosis: from imaging to biomechanics

According to the Global Status Report on noncommunicable diseases 2014, World Health Organization (WHO)1, stoke is the leading cause of deaths in the world (6.7 million per year) and the first cause of disability. It is more severe in China according to the recent Report on Cardiovascular Diseases in China 2014 (7 million stroke survivals and annually 2.5 million first stroke patients in China). Considering the large population suffering from hypertension (270 million) which is one of the main high risk of stroke, China is facing a tremendous challenge in stroke prevention.
It has been shown that carotid atherosclerotic diseases is responsible for about 25%-35% of ischaemic cerebrovascular events. In a population-based study with 5 million participants, 1/3 with age over 40 years old had detectable carotid intima thickening. Degree of luminal stenosis is the only validated criterion in current clinical practice. This criterion has been shown to have a serious limitation in assessing the risk of recurrent ischaemic events in the symptomatic cohort and it cannot identify the right individuals in the asymptomatic cohort. It is, therefore, an urgent need to find novel biomarkers.
Studies in the past 20 years have shown that atherosclerotic plaque is a multi-component structure and its morphological and compositional features have a closer relationship with patient clinical presentations and subsequent ischaemic events. However, these features have a low predicting power, in particular, for the asymptomatic cohort. An integrative approach with the combination of local and global features is needed in order to identify the right patient at the right time and provide the right treatment.
