Laparoscopic surgery: Any role in patients with failed in vitro fertilization cycles?

Objective: To investigate the fertility outcome of laparoscopic treatment in infertile women with repeated IVF failures.
Materials and Methods: This is a retrospective study conducted in a tertiary care, academic teaching hospital from January 2012 to December 2015. 45 infertility women with previously multiple implantation failures who underwent laparoscopic surgery were included in the study. Diagnostic laparoscopy and subsequent laparoscopic excision of suspected endometriotic lesions, lysis of adhesion were performed by the same skilled surgical team when indicated. Pregnancy and Delivery rates after surgery were recorded.
Results: Laparoscopy revealed pelvic pathologies in 44 patients. Endometriosis, tubal disease and pelvic adhesions were observed in 26, 14 and 15 patients, respectively. Following laparoscopy management, 24 patients (53.3%) delivered including 13 spontaneous pregnancies and 11 IVF pregnancies.
Conclusions: Laparoscopy in women with a history of recurrent IVF failures may detect unrecognized pelvic pathologies. Surgery performed by an experienced surgeon should be considered for these groups of patients.
